Neighborhood Rx Assistance Program

Houses of worship have an established role investing in the care of the sick. Bedside ministry, the traditional picture of church healthcare outreach, may be the most commonly understood model of healthcare ministry. However, churches today have the opportunity to reach beyond traditional pastoral ministry at the hospital bedside and into a pool of alternative models of healthcare outreach. These models respond to growing numbers of chronic illness, the decrease in insurance coverage among middle-income parishioners, and the growing sensitivity among evangelical churches toward the social needs of the community.

Faith Chapel's Neighborhood Rx Assistance Program is a new model of faith-based healthcare outreach.  It provides a team of trained volunteer health workers who will assist low- to moderate-income, uninsured, non-elderly adults seeking medication. 

Documentation - Before a person attends an appointment, they will be instructed to bring the following information:

  • Proof of age and residency (such as a driver's license, piece of mail, birth certificate, etc)
  • Proof of all sources of current income (such as a pay stub and/or last years tax forms).
  • A list of all medication prescribed (or the actual medication bottles), as well as a copy of all prescriptions. 

What do they need first? - The Neighborhood Rx Assistance volunteer should select the statement below that best describes a visitor's situation...

"I need to visit a doctor."

"I have a prescription which I need filled."
